In this paper we examine how the 1840 Vaccination Extension Act was passed in the Parliament and implemented by the Poor Law Commissioners and Boards of Guardians, ainly through the analysis of the Commissioners' Annual Reports . In our examination it will be clarifi ed that the promotion of vaccination under 1840 Act was steadily progressed, in spite of many impediments to it, such as the people's hate against Poor Law authorities and the absence of ompulsion under the act. We conclude that the vaccination, which went side by side Poor Law medical relief, was the first step to the national reventive medicine.
