A 67-year-old man with schizophrenia was admittedto our hospital complaining of nausea and legedema, and fell into a shock state the same day.The blood tests revealed high levels of the troponinI (5.2 ng/mL) and transthoracic echocardiographyshowed left ventricular (LV) dilatation and wallmotion abnormality, he was suspected of acute heartfailure caused by coronary artery disease. But coronaryangiography showed no significant coronaryarterial stenosis. The cardiac output was high andperipheral vascular dilatation occurred, he was treatedsupported by noradrenaline, but he passed awayon the third day. He did not receive supplement ofthiamine during this period without being given adiagnosis because he worsened rapidly. Myocardialnecrosis and colliquative myocytolysis were observedin the autopsy. The thiamine level at admission wasproven to be low, and was diagnosed as shoshinberiberi.
